Completing an Advance Care Directive (ACD) can be challenging. An Advance Care Directive empowers you to make clear legal arrangements for your future health care, end-of-life care, preferred living arrangements and other personal matters.
ECH are facilitating free workshops in November 2022, providing the opportunity for you to learn about Advance Care Directives and discuss your concerns in a supportive environment. There are three sessions, all of which need to be attended in order to formulate your own Advance Care Directive.
Workshop 1: Tuesday November 15th
Introduction to the principles of Advance Care Directives, and the importance of writing down and communicating your wishes about your future health and personal care in advance. We will also explore issues associated with appointing a substitute decision-maker and discuss the sort of information you might want to communicate about your values and wishes.
Workshop 2: TBA
In this session we will further examine how to express your values and wishes on the Advance Care Directive form. Bring discussion points, thoughts, and any questions you may have.
Workshop 3: TBA
The final session is an opportunity for you to clarify and express any concerns. A Justice of the Peace will be available to witness completed documents and there will also be a photocopy machine to make copies of the completed form.
